Review forms support three question types. Each question can be marked as required or optional.
LIKERT scale
The LIKERT type collects a numeric score on a fixed scale. The display style is configurable per question:

The score range is 1 to 5 by default. The low-score threshold for auto-ticket creation is configured globally for the form, not per question.
Single choice
The single-choice type presents a list of predefined options and allows the visitor to select exactly one. An optional "Other" field can be enabled to allow free-text entry when none of the options fit.
Multi-choice
The multi-choice type presents a list of predefined options and allows the visitor to select one or more. An optional "Other" field is available.

Questions are displayed in the order you configure them in the Review Management admin panel. Drag to reorder.
